Cherry Street East, West, & Warehouse Attendees must be enrolled. The following forms can be completed and submitted online. If you have any problems with these forms please contact our main office at 620-431-2161. For anyone wanting more information regarding attendance, please review our list of frequently asked questions before calling. Your child will not be accepted until ALL the forms for their grade level are completed.
